Reproduced the event in creative mode twice now, hitting the ceiling triggers it while the borders on the side are no issue. This happened on single player, PS5. It's not an anti-mesh thing it's an un-fixed bug. Flying into the ceiling has always caused deaths & lost stuff. It can also happen when flying into the sides although that's pretty rare.
