Help! Icons don't have description

I posted this in the bugs section so my apologies if this isn't allowed posting again in this section. 

I'm having trouble identifying the items in my inventory. All I see is the icon image and not the description of the item. Yes, even when highlighted or selected. For example, I'll have a berry selected and it does not show what type of berry it is.  Only after pressing RT (I think)  and consuming it, that I see whatever type of berry it was on the left side of my screen. It's the same for every item in my inventory so being new to the game, I'm trying to craft things and make shelters, and all that and it's difficult knowing which item is what by picture alone. Once created as well, it's difficult to know what item is what with no text description. 

I've noticed that on some online tutorials, the descriptions of items appear on the top of the screen and when used, that also appears on the top. I don't know if it makes a difference but I see that on the left side of my screen.

Any help or advice is greatly appreciated.

Go up to tool tip icon (looks like chat bubble with an i and 3 horizontal lines) and toggle it on. 

Should be one for inventory and one for things like smith, vault, etc

You might have hit the "Toggle Tooltips" button at the top of your inventory by mistake. Try pressing it again and see if it helps. See the attached image here.
